STEP 1- ASSESSMENT
It starts with an assessment over the phone; with you, your loved one, or a health care professional familiar with your loved one's needs.
- Health care needs based on the Activities of Daily Living (ADL's)
- Financial Requirements
- Geographical Region
- Personal Preferences
Based on this initial interview, we may request further one-on-one assessment with the client needing placement to determine the most appropriate level of care.
STEP 2- RESEARCH
Our trained coordinators begin examining potential care options to best match the client with residents, caregivers, as well to meet all needs and preferences identified during the assessment process.
STEP 3- OPTIONS & TOURS
We then present several options which have been selected based on your needs and requirements. We are available to set up and accompany you on tours of the communities we recommend and to assist you in the decision-making process. Many of our clients choose to have us along on tours to guide, obtain key information, and ensure that they are represented holistically to the care community.
STEP 4- MOVE-IN & COORDINATION
Once a care option has been selected, your coordinator will work with the selected facility to ensure that every detail of move-in is complete.
What Makes Us Different?
We review public disclosure files (PDF's) and have developed an extensive database in conjunction with several other referral agencies to quickly retrieve information about facilities. We use this information every day as part of the process to determine if a facility or care center meets our high standards to refer to you.
We are independently owned and operated with NO vested interest or ownership in any Long Term Care facility or senior housing company.
